Combined Tx ECMO and pMCS – where is there a benefit in Cardiac Surgery?

Prof. Potapov from Berlin (Germany) highlighted the upgrade of the ESC guidelines of temporary MCS from IIb to IIa, whereas IABP is not recommended any more. He introduced their in-house SOP on cardiogenic shock including two hard parameters (lactate > 8 mmol/l and long CPR) to differentiate which MCS device to use. The ECMELLA flow of up to 10L is essential for the survival during cardiogenic shock, as lung edema is an important complication in these patients (SCAI stage D). He also introduced his concept on a newly developed single arterial access model involving a Y-formed vascular graft for Impella and ECMO as axillary access.
